Chronic diseases are defined broadly as conditions that last 1 year or more and require monitoring and ongoing medical attention or limit activities of daily living or both. Chronic diseases such as heart disease, cancer, and diabetes are the leading causes of death and disability in the United States.

Dr. Brad Lofton and Kimberly Randolph pride themselves in managing a wide variety of chronic conditions, including diabetes, hypertension, high cholesterol, arthritis, heart disease, Alzheimer’s disease and dementia, COPD, and asthma.

We believe in a holistic approach to chronic disease where we not only focus on lifestyle-based therapeutic interventions in managing disease whenever possible, and empower the patient to take the driver’s seat. 

We also offer Chronic Care Management Services, where we are able to take care of your health in between your doctor’s visits by connecting with you in the comfort of your home. 

Chronic Care Management is a non-face-to-face service provided to patients who have multiple (two or more), significant chronic conditions. The service is covered by Medicare and some private insurers.
